Hypothyroidism is a clinical entity very commonly encountered in practices. Untreated hypothyroidism can lead to hypertension, dyslipidemia, infertility, cognitive impairment, neurological and neuromuscular involvements. Neurological complications include reversible cerebellar ataxia, dementia, peripheral neuropathy, psychosis, coma, encephalopathy (Hashimoto's encephalopathy), disorders. The disease identification the initial stage remains difficult as it presents with large number of features. Neonatal screening protocols all patients help timely detection well treatment. Most manifestations be reversed on administration levothyroxine which works even reversing symptoms reverses metabolic abnormalities. It important evaluate every patient or suspicion for any
